Must be Navigate to https://qainterrapt.brainster.xyz/login.
Must be Logged in.
*Steps to Reproduce*
1.Click on 'Attendees from navigation menu.
2. Click on the 'Create' button on the right top corner.
*Expected Results*
The example format for a phone should be shown next to or under the 'Phone' input field.
*Actual Results*
The example format for a phone is not shown.
