Steps to reproduce:

1.Open the mobile application
2.Log in with your account.
3.Browse and add items in the Cart
4.Click on the "Proceed to payment" button.
5.Choose the payment method as "debit card."
6.Click on the "Add" button in the "Add your card info" pop-up window.

Expected result:
After clicking on the "Add" button in the "Add your card info" pop-up window, a form should be displayed to enter debit card information

Actual result:
After clicking on the "Add" button in the "Add your card info" pop-up window, NO form is displayed to enter debit card information. The payment is processed as cash method
